剧集采用单元案件串联主线的模式，围绕狐爪伤痕、水鬼索命、傩戏泣血、巨灵流泪、古寺婴啼五大奇案展开。狄仁杰在巡查江南途中，与段十四（李彧 饰）、楚青岚（邵芸 饰）、林不言（单敬尧 饰）组成探案团队，以近妖之智探微毫，以天人之技窥人心，在迷雾交织的诡事中抽丝剥茧，侦破连环奇案，逐渐触及朝堂深处一只翻云覆雨之手。
